What to Include in a Tattoo Consent Form

September 20, 2026 ยท 5 min read

The short answer

A tattoo consent form usually confirms the client's identity and age, asks about health conditions, records their agreement to the risks, gets permission for photos, and ends with a signature and date. Rules vary by state and county, so check with your local health department.

1. Identity and age

Record the client's legal name and date of birth, and check a government ID. Most U.S. states require clients to be 18, and some allow minors only with a parent present.

2. Health disclosures

Ask about conditions that can affect tattooing or healing:

  • Heart conditions, diabetes, epilepsy, bleeding disorders
  • Blood thinners and other medications
  • Allergies (latex, pigments, ointments)
  • Pregnancy or nursing
  • Alcohol or drug use in the last 24 hours

3. Risks and permanence

The client confirms they understand tattoos are permanent and carry risks like infection, allergic reaction and scarring.

4. Photo consent

Ask before taking photos, and say how they'll be used โ€” client file only, or also portfolio and social media. Be especially clear for intimate placements like the chest, breast or genital area. A client should be able to say no to photos and still get tattooed.

5. Aftercare and signature

Confirm the client received aftercare instructions, then collect a signature and date. Store the signed form securely and keep it as long as your local rules require.
